Just as the name implies, the sign test is a non-parametric statistical procedure that evaluates the number of differences between paired comparisons using + and \u2013 signs to represent the direction of the differences between the pairs.\r\n\r\n<hr \/>\r\n\r\n<h4 style=\"text-align: center\">We can think of the sign test as a statistical method that compares pairs of outcomes and uses the (+) sign to describe the agreement and the (-) sign for the disagreement.<\/h4>\r\n\r\n<hr \/>\r\n\r\nConsider that we wish to compare the number of symptoms reported by concussed patients in two different groups.\u00a0 The first group will be comprised of individuals who are asked to sit quietly in their room for seven days following their concussion injury. The second group will be comprised of individuals that are asked to participate in seven days of controlled and monitored exercise beginning 24 hours after the individual is asymptomatic.\r\n\r\nGiven this scenario, the sign test can be used to test the null hypothesis in the two-group comparison, where the null hypothesis is written as:\r\n<p style=\"text-align: center\"><em>The chance for the number of symptoms in group 1 (the sedentary group) to be greater than the number of symptoms in group 2 (the exercise group) is equal to the chance for the number of symptoms in group 1 (the sedentary group) to be less than the number of symptoms in group 2 (the exercise group).<\/em><\/p>\r\n&nbsp;\r\n<div class=\"textbox textbox--key-takeaways\"><header class=\"textbox__header\">\r\n<p class=\"textbox__title\">The numeric value associated with the aforementioned chance is equal to 0.5 or one half and is written as:<\/p>\r\n\r\n<\/header>\r\n<div class=\"textbox__content\">The <strong>probability<\/strong> of (the number of symptoms in group 1) <strong>being<\/strong> <strong>greater than<\/strong> the <strong>probability<\/strong> of (the number of symptoms in group 2 )\u00a0<strong> is equal to<\/strong> the <strong>probability<\/strong> of (the number of symptoms in group 1) <strong>being less than<\/strong> the <strong>probability<\/strong> of (the number of symptoms in group 2 )\u00a0 <strong>is equal to one half<\/strong><\/div>\r\n<\/div>\r\n<p class=\"ABodyCopy\">$$p(scoregroup1 &gt; scoregroup2) = p(scoregroup1 &lt; scoregroup2) = \u00bd$$<\/p>\r\nWe can simplify the computations of the sign test by evaluating the outcomes using the binomial equation.\u00a0 The binomial equation is a formula that computes exact probabilities for an outcome with two possible options.\r\n<p style=\"text-align: left\">In this first example, when comparing symptoms between a patient in group 1 and a patient in group 2, the two possible outcomes are either to have more symptoms or less symptoms.<\/p>\r\n<p style=\"text-align: left\">In the <strong>SIGN TEST<\/strong> the outcome options are represented with a (+) or a (-) sign, and as such, the binomial equation can be used to estimate exact probabilities for any pairwise comparison.<code><\/code><\/p>\r\n<p style=\"text-align: center\"><img src=\"http:\/\/pressbooks.library.upei.ca\/montelpare\/wp-content\/uploads\/sites\/49\/2020\/04\/signfrm.png\" alt=\"\" class=\"aligncenter wp-image-468 size-full\" width=\"408\" height=\"87\" \/><\/p>\r\n<p style=\"text-align: left\">NOTE: The following is a quick overview of the elements of the binomial equation used to establish probability.<\/p>\r\n<strong><span style=\"color: #0000ff\">P<sub>x<\/sub> <\/span><\/strong>refers to the probability of exactly <em>x<\/em> events appearing in <em>n<\/em> trials. For example \u2013 the probability of turning up three heads in 5 tosses of a fair coin.\r\n\r\n<strong><span style=\"color: #0000ff\">p<sup>x<\/sup><\/span><\/strong> refers to the expected probability of the event associated with the x term on any given trial (if we are flipping a coin then this value is \u00bd with a fair coin).\r\n\r\n<span style=\"color: #0000ff\"><strong>q<sup>n-x<\/sup><\/strong><\/span> refers to the probability of an event on any given trial <em>q =\u00a0 1- p <\/em>(usually this value is \u00bd if we were flipping a coin).\r\n\r\n<strong><span style=\"color: #0000ff\">n<\/span><\/strong>\u00a0 refers to the number of events.\r\n\r\n<strong><span style=\"color: #0000ff\">x<\/span> <\/strong>refers to the number of a given outcome being evaluated (e.g. how many heads were you expecting in the total number of tosses).\r\n\r\nIn the application of the sign test, the binomial equation can be used to determine if the number of (+) signs occurs more or less often than the number of (-) signs.\r\n<h3><em><strong>Application 1:<\/strong><\/em><\/h3>\r\nConsider that you are responsible to test the efficacy of exercise treatment for concussion recovery at your rehabilitation clinic. You decide for the next 20 patients that arrive following a concussion, alternate assignment of the patient to either the sedentary group or the activity group. \u00a0After 7 days of treatment within their respective groups, you survey the patient to determine if the number of symptoms reported by the group assigned to sedentary therapy is higher (or lower) than the number of symptoms reported by the group assigned to the physical activity intervention.\r\n\r\nThe data are presented in the table below, and the sign test is used to determine if the p(symptom reports in group 1 &gt; symptom reports in group 2) = p(symptom reports in group 1 &lt; symptom reports in group 2) = \u00bd.\r\n\r\nIn the table, the number of symptoms reported by each pair of members from either the sedentary In this equation x refers to the number of patients from the sedentary group reporting more symptoms than the patients in the exercise group (x = 17), and the expected probability, based on the null hypothesis where we expected that the number of pairs of patients reporting more or fewer symptoms would be equal is p=0.5.\r\n\r\nIn the sign test, the test statistic is the number of (+) signs.\r\n\r\n<img src=\"http:\/\/pressbooks.library.upei.ca\/montelpare\/wp-content\/uploads\/sites\/49\/2020\/04\/signfrm2.png\" alt=\"\" class=\"aligncenter wp-image-476 size-full\" width=\"883\" height=\"422\" \/>\r\n\r\nBy convention, we typically accept the null hypothesis when the resulting <span style=\"color: #0000ff\"><strong>P<sub>x<\/sub><\/strong><\/span> is greater than 0.05. In this example where we compared the number of patients from the sedentary group reporting more symptoms than the patients in the exercise group, the probability is 0.001 suggesting that the null hypothesis is false and therefore can be rejected.\r\n\r\nIn SAS, we can evaluate the null hypothesis for this example using the following program. The problem with the application of the PROC UNIVARIATE procedure here is that the sample size is too small to meet the assumptions of normality and therefore computing the significance of the difference requires that we consider an alternative to the parametric tests. This result indicates that there is a significant difference in the responses of the two groups as determined by the SIGN test.\r\n<h3><em><strong>Application 2:<\/strong><\/em><\/h3>\r\nConsider that you have two samples of 10 individuals in each sample. You identify a group of 10 smokers and a matched group of 10 non-smokers -- and you are interested in determining if the outcome on the dependent measure (resting heart rate) is the same in both groups.\u00a0 You begin by matching the participants in your groups on the variables: age, sex, socio-economic status, education, and smoking status where you separate the groups according to: never smoked versus smoke at least 3 cigarettes per day.
